Please click here to download and print the Application Form. Complete and sign the form and return to us either by:
a) post or deliver the form and supporting documents to Modelex, 2 rue du Gabian, Monaco, 98000
b) scan and email the completed form and supporting documents to [email protected]
Your child’s identity photo must be full face with plain background and can be a JPEG (taken on your phone) sent by email with your application.
Please also pay the Application Fee of Euros 375 per child:
a) by cheque made payable to Modelex (drawn on a Monaco or French bank only)
b) bank transfer (quoting your child’s name) to:

Before returning your application, please ensure that you have completed and signed all forms, and then return them to us with school reports where applicable, photographs, identity documents, medical forms and vaccination certificates.
Only fully complete applications will be passed to the Admissions Team for assessment.
